EYE CLINIC OF BELLEVUE MEDICAL RECORDS CUSTODIAN INFORMATION
The Eye Clinic of Bellevue is no longer seeing patients after June 30, 2026 and is officially closing July 31, 2026.
After closing, your records from your time at Eye Clinic of Bellevue will be transferred to Overlake EyeCare, PS (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws.
If you wish to have a copy of your records sent to you or transferred to another provider, submit a written request to the Records Custodian. Requests should be directed as follows:
-There will be a transition period from the closure date to the time Overlake EyeCare, PS has access to our patient records. If you need your records during the transition period, you can download them from our patient portal. See our main webpage www.eyeclinicofbellevue.com for step-by-step instructions on how to do that.
-In accordance with applicable state law, you may be charged for the copying and transmittal of your records.
